How to use this system.
Online forms is an interactive system that uses your answers to questions to take you through the form(s). Each page looks like the one you are reading now.
When you are ready to move to the next question
Click on the button labelled "Next" on the bottom of the screen.
If you want to review earlier questions and answers
Click on the "Back" button on the bottom of the screen. You can go backwards and forwards through the screens. Do not click the “back” button in your browser.
If you leave the online form window open and have an active internet connection,
the online form will retain the information you have entered and will show you a summary at the end.
Do you need time to collect information?
The online form will keep your information as long as you leave the online form window open and have an active internet connection.
If you close the window your information will be lost. It is best to have all your information with you before you start.
Do you want to seek a fee deferral, exemption or waiver?
Fees must be paid to start an application. If you want to seek a fee exemption or waiver, or if the applicant is a corporation
that is entitled to pay fees at the natural person rate, you will need to lodge the application in person or by post.
You can still use this online form, however you will need to print it out and lodge a hardcopy with any supporting material.
